HomeInformation Technology ServicesWindows Admin Form Policy 463 Workstation Administrative Privileges Request Form Windows Administration Rights Purpose: University-owned computers are managed centrally by Utah Tech Information Technology Services and are not issued with employees having administrative privileges as per Policy 463. Providing administrative privileges is strongly discouraged by all leading cybersecurity frameworks, cybersecurity insurance underwriting, and internal IT audit requirements. The purpose of this form is to document the justification for an exception to University policy I/we, the undersigned, are requesting administrative user privileges for a University-owned and managed workstation. I/we understand and agree to the following: University-owned computers are the property of the University and use of these computers is subject to all applicable University Information Technology conduct policies (refer to Policies 461, 462, and 463). Administrative privileges are granted contingently for a justifiable business purpose and not for personal convenience. Administrative privileges may not be used to circumvent University policy, attempt to disable or remove University security/management tools, or to facilitate the use of University computers unrelated to the business justification listed below. The budget index code will be charged $15 annually (per user) to cover licensing costs for the University’s Privilege Access Management (PAM) software that manages administrative privileges. The Information Security Office may revoke administrative privileges for misuse of the privileges or in the event of a security incident caused by the use of administrative privileges.
